Asking for Information
When was the last time you updated your medical records?
GP Practices are required, as part of looking after your health, to ask patients to provide certain medical information from time to time.
Please help us keep your medical records up to date by responding to such requests.
We will only ask you for information that we need to make sure your records are complete.
Some of our funding is dependent on us having this up-to-date information which enables us to employ sufficient staff to manage your health needs.
